What information do I need to have to pre-register on line?

Before you begin the online pre-registration process, you will need:
• Procedure date and time
• Physicians name
• Type of procedure (such as Inpatient, Outpatient Surgery, Maternity Services)
• Personal contact information
• Your insurance identification card

When should I pre-register on-line?
 Your physician should have provided you with information about our on-line pre-registration process. You can begin your on-line pre-registration anytime after your doctor’s visit, but no later than 48 hours prior to the date of your scheduled procedure. If you have less than 48 hours before the time of your procedure, you can pre-register over the telephone by calling our Admitting and Registration Office at 209-564-5101, between the hours of 11:00am – 5:00pm, Monday through Friday.

What happens after I submit my pre-registration information on-line? After you have submitted your online pre-registration form, you will be contacted by our admitting representative by phone, within two business days. It is very important that you list your current phone numbers to be reached at during the normal business day.

What is the benefit of pre-registering on-line?
By completing this form online, you are submitting your personal information to assist in your pre-registration prior to your arrival at our facility. Our goal is to make your arrival at our facility as smooth as possible.

Do I still need to come to the facility prior to the date of my scheduled procedure?
Yes, if your physician informed you that you will need to have pre-operative exams prior to your scheduled procedure, you will need to come to the facility 48-72 hours prior to the actual date of your scheduled procedure/surgery. Please note that there will be forms that will require your signature when you arrive to Pre-register and on the day of your procedure.

Where do I go for my pre-operative exams?
This will depend on what campus your procedure has been scheduled at. If you are scheduled at Mercy Medical Center, you will need to report to the Main Admitting department located at 333 Mercy Ave.. If you are scheduled at our Mercy Medical Pavilion, you will need to report to the registration department located at 315 Mercy Ave. Our facility prefers that you arrive for your pre-operative visit between the hours of 11:00am – 5:00 pm, Monday – Friday.
